1.      All groups using the facilities must provide a sufficient number of adults to ensure order and to protect against loss or damage. The person who signed the application for use of the facilities is financially responsible for any loss or damage the facilities may sustain as a result of carelessness, negligence, or insufficient supervision of his/her group.
2.      A responsible person and the wedding coordinator must be present for weddings.
3.      No rice or grain may be thrown in or on the church grounds.
4.      Alcoholic beverages are not permitted on the premises under any conditions.
5.      Smoking is not permitted inside the building.
6.      Facilities furnishings and equipment are not permitted to be removed from the church property unless prior approval has been obtained from the chairperson of trustees, pastor, or the program director. Approved members or groups must sign them out in the office and will assume responsibility for any damage.
7.      Facilities must be left in a reasonable state of cleanliness, as found. Tables and chairs need to be returned to original set up. Trash should be deposited in trash receptacles.
8.      Only dripless candles may be used in the Sanctuary.
9.      Although no separate charge is made for use of the nursery by organizations while meeting at Faith United Methodist Church, there are certain requirements which must be followed. They are:
1. No children may be left in the nursery without supervision. Such child care must be provided and paid for by the group or organizations utilizing the facility.
2. All toys, etc., must be replace on the shelves.
3. Improper use of equipment is prohibited, e.g., older children in cribs.
4. Parents are expected to furnish their child's diapers. Diapers in the nursery are for emergency use only.
5. The cost of replacing any broken or missing toys or equipment will be charged to the organization or individual.

     Save it, open it, and print it. Fill it out and deliver it to the Administrative Assistant at the chuch office to be checked over and dated to begin the process of approval.
